AMCS – Help at home

If you need support and assistance to continue living independently in your own home, a Home Care Package might be right for you. Home Care Packages are services for seniors that can assist you with your daily living needs. Home Care Packages are coordinated and tailored to meet your specific needs and goals, giving you choice and flexibility. The services may include assistance around your home, personal care, transport and meal services to name a few examples.

How are Home Care Packages provided?

We will work with you and your chosen representative in partnership to create a care plan and budget based on your individual goals. Your care advisor will monitor your budget and review needs in accordance to your care plan.

Who can access AMCS Home Care Packages?

If you are aged 65 or more you can access a Home Care Package but you will need to be assessed first by an Aged Care Assessment Service (ACAS). The ACAS team will talk to you about how well you are managing at home, and then provide you with a letter that tells you what level of home care package you have been approved to receive.

How can I be assessed by the
Aged Care Assessment Service?

You can contact My Aged Care directly to be referred to ACAS for an assessment. A referral can also be made by your doctor or community care service provider.  If you want us to support you with this, please call us on 03 9689 9170 or email us using our contact form above.

What will a Home Care Package cost?

The Australian Government pays for most of the cost of aged care in Australia. You may be asked to contribute towards the cost of your care if you can afford to do so, however you will never be denied the care you need because you cannot afford it.

Any fees will be discussed during the care planning process and the agreed amount will be documented in your agreement with AMCS.
