Employment Opportunity – HCPP CARE ADVISOR

AMCS currently have an exciting position available for the right candidate.

This autonomous role as a HCPP Care Advisor will focus on the writing of detailed case notes and identifying tailored care solutions to enable our clients to achieve their goals and enhance their quality of life. You will aim to make a positive difference to families, achieve the client’s satisfaction and build professional relationships.

The position will also require you to oversee the client’s package budget as well as connect professionally with family members and external organisations to serve the client’s best interests.

You will be regularly travelling to meet clients in the community and liaising with the relevant stakeholders, to ensure smooth delivery of care services.
