AMCS delivers a range of aged care services to the Polish and associated communities. The programs are delivered in such a way as to enhance independence and promote the ongoing wellbeing of older members of the community.

Community Care Program

The Community Care Program offers a range of home based services through:

Community Aged Care Packages

 A planned and coordinated package designed to assist people who require management of services due to their complex care needs. The aim of the program is to maintain the independence of elderly people living in the community for as long as possible. The program is for elderly Polish speaking people and for elderly Eastern European people (in the Western, Northern and Eastern Regions). APCS also provides a CACP program in the City of Greater Geelong. Services provided include: Case Management; Personal Care; Domestic Assistance; Meal Preparation; Transport; Home and Garden Maintenance; Respite; Social Support and Companionship.

Disability Services

We are able to provide a range of home based and social supports to people with a disability wishing to purchase these services.

Multicultural Brokerage & Support Services (MBSS)

The MBSS aims to maintain and support the independence of older people and people with disabilities living in the community for as long as possible. AMCS operates the MBSS and offers a quality service that meets the Common Community Care Standards and enables clients in receipt of Community Aged Care Packages (CACP), Extended Aged Care at Home (EACH) and Extended Aged Care at Home - Dementia (EACH-D) packages to access services that are tailored to meet the diverse linguistic and cultural needs of the elderly living at home or in siupported and unsupported accommodation in the metropolitan regions of Melbourne and the City of Greater Geelong. Services are also provided to individuals purchasing disability support services.

AMCS care workers are highly trained in meeting the sometimes complex needs associated with the care of older people with limited English language skills living in Victoria after having immigrated many years ago from various European countries.

Some of the specific services include: general CACP and EACH direct care with a focus on culture and language, support with medical appointments and basic translation (professional interpreting services still need to be accessed when appropriate), culturally sensitive personal care and culturally responsive meal preparation and presentation.

Volunteer Coordination - Friendly Visiting Program (FVP)

Polish-speaking volunteers are available to provide companionship to isolated elderly people or people with disabilities who are living in their own homes (Western and Northern suburbs). APCS also welcomes volunteers to assist in office activities.

Volunteer Coordination - Community Visitors Scheme (CVS)

Polish-speaking volunteers visit isolated residents in aged-care facilities to offer companionship and friendship. Facilities can make a referral for a volunteer to AMCS for isolated Polish residents.

Polish Older Persons Outreach Program

The main aim of the program is to reduce isolation within the Polish community in Victoria, improve links between the community and the Polish HACC target group, provide support and thus reduce early or inappropriate admission to residential care.

Planned Activity Group (PAG)

Centre-based support service and respite for the Polish speaking frail older people and people with disabilities. Activities include outings, craft, entertainment and exercises. This program operates on Mondays and Wednesdays at the Castley Hall Community Centre, West Sunshine and on Fridays in Geelong.

Polish HACC Access Program

A state-wide program which aims to increase access to appropriate services by Polish older persons, younger people with a disability and carers. This is achieved through provision of information, advocacy, needs identification and service development.

Supported Access Pilot Project

This project provides support to HACC eligible frail older people from a Polish speaking background who require support to access a range of services. Support provided includes provision of information, referral, advocacy and support to attend assessments if required. The program operates in the southern and eastern metropolitan regions of Melbourne.

Community Partners Program (CPP)

The aim of this program is to identify and address specific barriers in accessing aged care services and develop partnership between the Polish community and aged care services. The program enables people of Polish background to make informed decisions about aged care choices.
